Family Care Teams: Chronic Disease Management in Family Care Teams Chronic Obstructive Pulmonary Disorder (COPD)

This 1-hour webinar for Family Care Teams focused on improving chronic obstructive pulmonary disease (COPD) diagnosis and management through the Comprehensive Respiratory Care Program. The session began by acknowledging the traditional territories of Indigenous groups where Memorial University is located and emphasized the importance of accurate COPD diagnosis and evidence-based management strategies. Speakers highlighted significant challenges in Newfoundland and Labrador, including limited respirologist availability and long wait times for consultations. The program’s goal is to enhance patient education and access to care by linking acute care and community services for respiratory patients over a full year. Respiratory therapists provide home visits and virtual assessments, ensuring patients receive individualized action plans for managing exacerbations and health directives. Enrollment in the program has led to notable reductions in emergency visits and hospital admissions. Education on inhaler techniques remains critical, as more than half of patients demonstrate errors. Comprehensive respiratory care also includes collaboration with family care teams to streamline patient management.
